You are required to enter the MsSQL database server IP address, port number, Authentication mode, username and the database password.

Server name : <IP address>,<port number>

Authentication : SQL Server Authentication

Enter <username> and <password> in the respective fields as provided to you.

The default port is 1433. However most of the servers are configured on alternate ports for security reasons say 4433.
